As a foster care provider for Mountains’ Humane Society, I agree to the following statements:

I fully understand that the animals(s) shall remain the sole property of the Mountains’ Humane Society.(required)

I agree to return said animal(s) upon request if I am no longer able to care for them adequately.(required)

I agree to provide the animal(s) with good, loving care, including, but not limited to food, clean water, shelter and medication (when required).(required)

I agree to keep my foster pet(s) on a leash at all times when in public, on walks and bathroom breaks if I don’t have a yard.(required)

I agree to keep an ID tag on the foster pet(s) at all times with MHS number.(required)

I understand and acknowledge that I do not have any right or authority to keep, place or adopt out my foster animal(s) in other homes or with other individuals without MHS approval.(required)

I certify that my own pet(s) are currently licensed and up to date on their vaccinations (including rabies).(required)

I agree to keep my pets(s) separated from the foster pet(s) for at least 10 days. If the foster pet(s) is/are incubating an illness, this separation will minimize the chance of my pets getting ill. Exception: If the healthy pet was relinquished from the owner and was currant on all vaccinations or was boarded the 10 day quarantine does not apply.(required)

Should my foster pet(s) become ill, I agree to immediately call MHS and transport my foster pets to the designated veterinarian as soon as possible.(required)

I will instruct any interested parties (including myself) to contact MHS for an adoption application.(required)

I will transport my foster pets to and from MHS public events.(required)

I am willing to assist with home visits of prospective adoptive homes. And to follow MHS requirements.(required)

I will not publicize my foster pet(s) on any internet websites, Facebook or post any flyers.(required)

I will notify MHS at least 2 weeks before (Holidays at least 2 months in advanced) when I am planning to vacation, so another foster home, pet sitter or boarding arrangements can be made for my foster pet(s).(required)

I understand MHS must approve any pet sitter or boarding facility.(required)

I have received the Foster Care Guidelines and Requirements which I will retain and follow the guidelines set forth on it.(required)

I understand MHS is not liable for any damages that may be caused by a MHS foster animal. (i.e. chewed up clothing, furniture, carpet, potty accidents, injuries, etc.).(required)
