MHS K9 Adoption Application

The first step in adopting an animal from our organization is completing our Adoption Application. You must be 18 years or older to complete an application and apply to adopt. Our application is lengthy and detailed for a reason. Every question is important. Our goal is to match every animal with the right home. All of our animals are placed in approved homes only.

Please be sure to answer all questions completely or your application may not be approved. If a question is not applicable, please put NA.

You will be contacted within 2 weeks if you are being considered for the animal you are applying for. We do not adopt out of state at this time.
